Dog License Application (Webform)
- The City requires all dogs 5 months and older, to be licensed every year & to have a current rabies certificate.
- Dogs & cats are regulated by WI Statutes 174.05 - 174.10 & Section 12.10 of the Dodgeville Municipal Code.
- No more than 2 dogs and no more than 2 cats shall be kept in any household in the City of Dodgeville.
- A $25.00 late fee per animal will be applied to each license renewal if renewal tags are requested after March 31st.
Before completing this form, please ensure you have the following handy:
- Proof of Valid Rabies: State Law and City Ordinance requires that your animal has a current rabies vaccination. When submitting this form, you will need to attach a copy of the rabies certificate for each animal you are registering. This rabies certificate must include the Vet Name, Vaccine Manufacturer, Vaccination Serial Number, & Vaccine Expiration Number. Animal tag(s) will not be issued unless proof of rabies has been provided.
- Proof of Payment: Cash, checks, debit and credit card payments are acceptable.
- Please make all checks payable to: City of Dodgeville and indicate "Pet Licensing" in your memo line.

- Create an online account or pay as guest.
- If you choose to pay as guest, click the "Pay Now" button under Pay as Guest
- Select Licenses & Permits from the department drop down
- Select dog license (fixed or unfixed) in the payment type.
- Enter customer name
- Enter customer email
- Enter amount due.
- Enter in comments, if needed
- Select payment method
- Enter payment information
